Diurnal Blood Pressure and Heart Rate Variability in Hypertensive Patients with Cerebral Small Vessel Disease: A Case-Control Study

چکیده
BackgroundWhether autonomic dysfunction contributes to cerebral small vessel disease (CSVD) remains unclear. This study aimed explore the relationship between CSVD and blood pressure variability (BPV) heart rate (HRV).MethodsThis case-control recruited 50 patients with non-CSVD hypertensive age- gender-matched controls. All participants completed a 24-h ambulatory electrocardiogram recording BP monitoring (ABPM). Differences in HRV BPV two groups were examined. indices assessed by ABPM included mean systolic (SBP), diastolic (DBP), coefficient of variation weighted standard deviation SBP DBP.ResultsCSVD had significant higher daytime SBP, nocturnal DBP (P < .05 for all). lower fall compared controls (median: 1.0 versus 6.2, respectively; P .001) more likely be non-dippers reverse dippers. There no differences variables groups. Five logistic models built correlations CSVD. separately entered into regression models, together hyperlipidemia, ischemic stroke history, current use anti-hypertensive agents, serum urea nitrogen. In 1−3, significantly correlated (r2 = 0.308−0.340). model 4, was negatively (odds ratio [OR] 0.871, 95% confidence interval [CI] 0.804−0.943; .001), r2 0.415 fitting model. 5, pattern dipping associated CSVD, non-dipper (OR 8.389, 95%CI 1.489−47.254; .016) dipper 27.008, 3.709−196.660; having highest risks 0.413).ConclusionsLower is Non-dipper have risk

circadian blood pressure variability in normo and hypertensive diabetic patients
background: diabetic patients have a higher prevalence of non-dipping pattern in blood pressure (bp) than general population. non-dipping arterial pressure pattern is associated with increased cardiovascular risk. the objective of this study was to investigate the association between the clinical and paraclinical characteristics of the diabetic patients with circadian bp variability. m aterials...

Background: Hypertensive heart disease (HHD) is the constellation of abnormalities characterized by cardiac complications. Despite advancements in management of hypertension and access to medical care, incidence of HHD is an alarmingly increasing through time. However, information on determinants of HHD studies is limited in Ethiopia. We assessed determinants of HHD among adult hypertensive pat...

متن کاملBlood pressure gradients in cerebral arteries: a clue to pathogenesis of cerebral small vessel disease
RATIONALE The role of hypertension in cerebral small vessel disease is poorly understood. At the base of the brain (the 'vascular centrencephalon'), short straight arteries transmit blood pressure directly to small resistance vessels; the cerebral convexity is supplied by long arteries with many branches, resulting in a drop in blood pressure. Hypertensive small vessel disease (lipohyalinosis) ...
